At FORM, we are committed to your peak performance; if your gear doesn't meet your expectations, you can return or exchange it within 30 days of purchase. All items must be in their original, unworn condition with tags attached to qualify for a full refund.

Return Process

  Initiate Your Request

  Visit our online returns portal and enter your order number and zip code to select the items you wish to send back.

  Package Your Gear

  Securely pack your items in their original packaging and include a copy of your packing slip to ensure faster processing.

• 3.
  Attach the Label

  Print the prepaid shipping label provided via email and affix it clearly to the outside of your return package.

• 4.
  Drop Off and Track

  Leave your package at any authorized carrier location and keep your tracking number handy until your refund is confirmed.
